Patient Assistance Programs (PAP's). These programs help patients maintain
their health and well being by offering prescription drugs at either no cost or a small co-payment for the patients
who qualify. Unfortunately, though these programs have been around for years, they are often unheard of by the people
who need them most because no one has a vested interest in promoting them. Also, most Patient Assistance Programs
involve a large amount of red tape that prohibits most people from utilizing them. Still, when used properly, many
Seniors and low-income families can get their prescriptions for little or no cost.

Upon approval of your Membership Application, you will be mailed a Membership/Pharmacy Discount Card that you or any member
of your family may use at over 45,000 pharmacies nationwide. Soon after, you will be contacted by your
Personal Care Advocate.
Your first conversation will be a brief orientation. Your Personal Care Advocate will review your specific situation with
you and explain the Patient Assistance Programs that best suit your specific needs,
review the process with you and answer any questions that you may have. If you have any special needs that need to be
considered when working with us, this is a good time to let your Personal Care Advocate know.

Before your first conversation,
your Personal Care Advocate will have already matched all of your prescription drugs to the
specific programs available for that medicine, pre-qualified you for that program based on the latest available
information and completely prepared the paperwork for you to participate in each program.
Your forms for your initial prescriptions will be given to completely filled out with
the information you have given us except for the signature of you and your doctor. You will need to sign the forms and have
your doctors sign them and return them to us as soon as possible.
Once we have checked the forms for 100% accuracy
(any less can cause severe delays) we will forward them to the appropriate pharmaceutical company sponsored program and begin
managing your renewal process.
Within about 2-6 weeks, you should receive a 90-day supply of your medicine from the pharmaceutical company(s).
Your Personal Care Advocate will contact you when it is time to begin your renewal process. In most cases, the entire process
must be repeated once every three months.
If your doctor changes or makes additions to the medicines that you are taking you must contact your Personal Care Advocate
immediately for assistance. Care Advocates are available Monday through Thursday from 8am to 8 pm CST and on Friday from 8am to 5pm.
