- You will need proof of income when you receive your forms. In most cases this will be a
Social Security statement or benefit verification letter. If you do not have one you can call
the Social Security Administration at (800) 772-1213 to receive a copy.
- Mail your completed application to SelectCare Benefits Network with a $50 one-time
application fee.
- Include a $20 processing fee for each prescription.
- Upon approval of your application SCBN will send you completed paperwork for you and your
doctor to sign and return to SCBN.
- In most cases, medications will be delivered to the physician's office. However, some
pharmaceutical companies will deliver directly to the patient and some will provide a card
that the patient can take to a local pharmacy to pick up their medication.
- Normally every three months steps 3 through 5 must be repeated to reorder your medications.
- Continue to take your medications as normal.
If approved, you will generally receive your medications in 2 to 6 weeks. However, as SCBN never
dispenses medication or approves an application, we cannot guarantee the time of shipment of your
order. You should continue to get your prescriptions in the normal way until your order arrives.
- No additional purchase is required.
Although our service is marketed mainly through professional insurance agents, SCBN is wholly separate
from any other products or services that may be offered to you. You are never required to purchase any
insurance or participate in any other programs in order to take advantage of the services offered by SCBN.
- You are paying for a service.
The SelectCare Benefits Network is not an insurance company, and this is not insurance. You are paying for a
service, which is to assist you by researching your prescriptions and helping to complete the necessary forms to
qualify for manufacturer Patient Assistance Programs. Your money will only be refunded if we are unable to help you
(if there is no program for your medicine or if you do not qualify for it). It is not our policy to refund those who
simply change their minds or decide not to submit the forms once we have done our part.
- This is not the last step of the process.
You will receive completed application forms for your drug manufacturer's Patient Assistance Programs. You will need
to sign and have your doctor sign these forms and return them to SCBN for final processing.
